Step 1: Print and complete a Team eligibility form
Print and complete an
Interscholastic Team Eligibility form
for EACH team your school or club is entering the the SW IS Championship.
Complete the forms in pen and bring them to the event.
Step 2: Register each individual participant
Pre-registration is required. Register each individual using the links later on this page.
The number of registatrations allowed for this venue is NOT limited by the landowner. NTOA has
placed limits to keep event to a manageable size. Those limits are high enough that they have
historically allowed everyone to participate.

Save time on event day! Download and complete the waiver now.
Complete the waiver in pen and bring with it you to the event.
NOTE: All youth under 18 years of age must have a waiver signed by their parent or legal guardian.
