Each team
member must also provide a completed individual online entry form and download
and sign a waiver. If possible, all waivers from a team or
school should be submitted together with this team eligibility form.
Eligibility Certification
This certification must be
signed by a school official / teacher accompanying the team. If no school
official/teacher is attending the event, the official team coach may sign on
behalf of the school.
I hereby certify that the
team members entered in the 2010 S.W. Interscholastic Orienteering Championship
from the above named school/club meet the criteria set
forth on the NTOA website, including, but not limited to:
1.
For
School Teams:
- Members are enrolled full-time at the
above named school.
- Members can show school ID or we are
providing a letter from the principal stating that the students are enrolled
full-time in the above named school.
2.
For
Club Teams:
- All team members are primary members
of the above named USOF sanctioned Orienteering Club
- All team members meet the grade
requirements for the class in which they are competing.
- All team members are enrolled in a
public or private school or are home schooled.
- None of the members are enrolled
full-time in a school for which a team is competing in the Varsity or Junior
Varsity team competition.
3.
All
team members are members in good standing with the school or club for which
they represent.
